Job Description is as follows:                                                                                                                        Reports To: Charge Nurse

General Position Summary:

The Certified Nursing Assistant (CNA) assists the licensed nurse in functions as designated.  Works in close cooperation with other team members to give personal care to assigned residents. Assists in any way possible to provide a safe and comfortable environment for the residents, maintaining dignity, privacy and resident rights. Assists nurses working with assigned residents to administer to their personal needs: bathing, daily hygiene, toileting, lifting, turning or repositioning with the nurse. Feed residents as needed during meals. Perform other related duties as trained and assigned by the charge nurse. C.N.A.’s for the Oregon Veterans’ Home will be able to perform the following Essential Functions and meet all job requirements for any wing assignment throughout the facility.

Essential Functions / Major Responsibilities (OAR Division 63):

1.       Under the supervision of a licensed nurse, the C.N.A. may only provide care and assist residents with the following tasks:

a)       Tasks associated with communication and interpersonal skills

a.       Answering and placing call lights

b.       Coaching and mentoring other nursing assistants

c.       Communicating with residents and co-workers

d.       Maintaining confidentiality

e.       Reporting abuse, mistreatment and neglect

f.        Utilizing de-escalation techniques

c)       Tasks associated with infection control and Standard or Transmission Based Precautions:

a.       Assisting with coughing and deep breathing

b.       Bedmaking and handling linen

c.       Caring for the resident’s environment

g.       Implementing precautions associated with communicable and infectious diseases

h.       Implementing neutropenic precautions

i.        Maintaining resident cleanliness and grooming

a.       Implementing bleeding, cervical, hip and sternal precautions

b.       Moving and transferring residents

c.       Transporting clients in wheelchairs and specialized chairs

e.       Utilizing lifts and safe resident handling devices

f.        Turning oxygen on an doff or transferring oxygen between wall and tank at pre-established flow rate for stable residents

g.       Managing hazards in the workplace

j.        Performing cardiopulmonary resuscitation (CPR)

e)       Tasks activities of daily living (ADLS)

a.       Assisting with nutrition and hydration

a.       Measuring and recording height and weight

g.       Utilizing techniques for assisting with eating

a.       Administering bowel evacuation suppositories that are available without a prescription

c.       Assisting with the use of bedpan or urinal

e.       Collecting specimens:  sputum, stool and urine including clean catch urine specimens

f.        Providing catheter care including the application of and removal of external urinary catheters

g.       Providing ostomy care for established, healthy ostomy including cleaning the ostomy site and emptying the ostomy bag or changing the dressing or ostomy appliance or bag

h.       Providing perineal or incontinence care

c.       Assisting with personal care

a.       Bathing

b.       Providing comfort care

c.       Dressing and undressing

d.       Grooming to include: application and care of dentures, eye glasses and hearing aids

e.       Nail care for fingernails and toenails of persons with uncompromised circulation

f.        Oral hygiene

g.       Personal care considerations for persons who have tubes or special equipment

h.       Shampooing and caring for hair

i.        Shaving

j.        Skin care to include:  application of non-prescription pediculicides; application of topical barrier creams and ointments for skin care; maintenance of skin integrity; prevention of pressure, friction and shearing; and use of anti-pressure devices

d.       Assisting with positioning devices and restraints

e.       Assisting with restorative care

a.       Ambulating

b.       Applying, turn on and off, sequential compression devices

c.       Assisting with and encouraging the use of self-help devices for eating, grooming and other personal care tasks

d.       Assisting with bowel and bladder training

e.       Assisting with ADL programs

f.        Assisting with the use of crutches, walkers or wheelchairs

g.       Assisting with warm and cold therapies

h.       Caring for, applying and removing antiembolus stockings, braces, orthotic and prosthetic devices

i.        Elevating extremities

j.        Maintaining alignment

k.       Performing range of motion exercises

l.        Reinforcing task sequence by breaking down tasks into small, obtainable steps

m.     Reinforcing the use of an incentive spirometer

n.       Using footboards

o.       Utilizing devices for transferring, ambulation and alignment

f.         Tasks associated with collaboration with health care team

g.        Tasks associated with observation and reporting

a.       Observing and reporting changes in condition to licensed nurse

b.       Measuring and recording:

                                                                                      i.      Temperature, apical and radial pulse, respiration and blood pressure (manual and electric – forearm, lower leg, thigh, upper arm and orthostatic BP readings)

                                                                                    ii.      Emesis

                                                                                  iii.      Liquid stool

                                                                                   iv.      Pain level using a facility approved pain scale

                                                                                     v.      Pulse oximetry

                                                                                   vi.      Urinary output, both voided and from urinary drainage systems

h.       Tasks associated with documentation

i.        Tasks associated with end of life care

2.        The CNA must be regularly supervised by a licensed nurse and under no circumstances work independently without supervision or monitoring by a licensed nurse who provides assessment of residents as described in OAR 851-063-0030(3)(a)(b)

a.       Standards of care for CNA’s.  In the process of resident care, the CNA shall consistently:

                                                              i.      Apply standard precautions according to the Centers for Disease Control and Prevention (CDC) guidelines

                                                            ii.      Use hand hygiene between episodes of care

                                                          iii.      Use appropriate body mechanics to prevent injury to self or resident

                                                           iv.      Follow the care plan as directed by the licensed nurse

                                                             v.      Use appropriate communication with resident, resident representatives and friends and with co-workers

                                                           vi.      Use alternatives to physical restraints or apply physical restraints as directed by the licensed nurse

                                                         vii.      Determine absence of pulse or respiration and initiate emergency response

                                                       viii.      Report to the licensed nurse any recognized abnormality in resident’s signs and symptoms

                                                           ix.      Record observations and measurements, tasks completed and resident statements about condition or care

                                                             x.      Apply safety concepts in the workplace

                                                           xi.      Report signs of abuse, neglect, mistreatment, misappropriation or exploitation

                                                         xii.      Demonstrate respect for rights and property of residents and co-workers

                                                       xiii.      Maintain resident confidentiality

Supervisory Responsibility:

May be asked to mentor CNA students or newly hired CNA’s.

Interpersonal Contacts:

1.       Professional contacts with OVH Employees, Department Directors, Physicians – in person and via telephone.

2.       Professional contacts with residents and resident representatives – in person and via telephone.

Education and Experience:

Graduate of an accredited Certified Nursing Assistant Training Program and current certification with the Oregon State Board of Nursing.

Job Conditions:

1.       Position is within the Oregon Veterans’ Home with ability to move throughout the facility, required to work on any unit assigned.

2.       Position is a 7 ½ hour shift. Normal working hours may vary to insure adequate staffing.

Essential Physical Elements:

1.       Must be able to stand for long periods of time during a 7 ½ hour shift.

2.       Must be in good physical and mental health, sufficient to permit full performance of the duties of the position.

3.       Must be able to lift, bend, stoop and stretch as required to lift, move and/or transport residents as required in daily routine.

4.       Must be able to manage pushing moderately heavy weight, i.e., kitchen carts and residents in wheelchairs, some residents weighing up to 350 lbs.

5.       Must have the ability to lift up to 30 lbs. up to 10 times periodically during a 7 ½ hour shift.
